- Be specific. Dates, locations, names, meeting titles, case numbers, and links help us verify information.
- Attach supporting material. Public documents, photographs, video, and screenshots can help establish what happened. Only send material you are authorized to share.
- Protect sensitive information. Do not use this form for emergencies, passwords, Social Security numbers, banking details, medical records, or other highly sensitive personal information.
- Tell us how to reach you. Contact information is optional, but it may be difficult to pursue a tip if we cannot ask follow-up questions.
